Pupilloplasty in combination to a femto-cataract-surgery for a traumatic cataract and mydriasis A football player came to our department 10 years after having an eyeball contusion. He had a traumatic cataract and a large atonic pupil. Our plan was first of all to do a cataract surgery using the femtosecond laser as an assistance, as we didn't know if some zonular weakness would be present.Some minimal zonular weakness was found, so a CTR was implanted together with a one piece acrylic IOL in the bag. After that, OVD was used once again and a pupilloplasty was done. Our target was about 4mm, which is cosmeticaly accepted and also "enough" in terms of managing postoperative complications. If a retinal detachment occurs, one should be able to put an early diagnosis and treat it surgically without any limits. We had luck on that, nothing of it happened. The surgery was done under some sedation, using conjuncain drops and intracameral lidocaine. The result is beautiful in my opinion, the patient had a 20/20 uncorrected vision on day one. This is not a method I invented, so here come some credits:
